A Research Guide to the Ancient World: Print and Electronic Sources is a partially annotated bibliography that covers the study of the ancient world, and closes the traditional subject gap between the humanities and the social sciences in this area of study. This book is the only bibliographic resource available for such holistic coverage.

So, on 22 July 1865, under the title ‘Philosophy and Punch’, did England's premier comic weekly greet the election of J. S. Mill as MP for Westminster. Mill held his seat for only one term, until the general election of 1868, when his Whig-Liberal colleague Robert Wellesley Grosvenor was re-elected, but Mill was replaced by the loser in 1865, the Conservative W. H. Smith, Jr., who, though he never went to sea, became the ruler of the Queen's navy. The reasons (...) for that reversal have engaged the attention of many, including Mill himself; I should like to introduce into the discussion material from an ignored source, the comic weeklies, which took a continued and close look at Mill's behaviour during his parliamentary years. While this evidence generally does not disconfirm earlier judgments—including my own— it does more than merely add to the induction. First, it shows how different political stances led journals to focus on different aspects of Mill's parliamentary career, and to adopt different rhetorical strategies in portraying him in picture and word. Second, it demonstrates how the hardening of party allegiances during the parliament of 1865–68, which accelerated in the preparatory campaigns for the general election of 1868, affected Mill adversely. Third, it suggests strongly that it was not his ‘crotchets’ or ‘whims’, especially women's suffrage and proportional representation, that damaged his chances for re-election, but his advocacy of causes unpopular with the majority of Liberals as well as with Conservatives. ObjectiveMost young adults diagnosed with breast or gynecologic cancers experience adverse reproductive or sexual health outcomes due to cancer and its treatment. However, evidence-based interventions that specifically address the RSH concerns of young adult and/or LGBTQ+ survivor couples are lacking. Our goal is to develop a feasible and acceptable couple-based intervention to reduce reproductive and sexual distress experience by young adult breast and gynecologic cancer survivor couples with diverse backgrounds.MethodsWe systematically adapted an empirically supported, theoretically grounded couple-based intervention to address (...) the RSH concerns of young couples coping with breast or gynecologic cancer through integration of stakeholder perspectives. We interviewed 11 couples with a history of breast or gynecologic cancer to review and pretest intervention materials. Three of these couples were invited to review and comment on intervention modifications. Content experts in RSH and dyadic coping, clinicians, and community advisors participated throughout the adaptation process.ResultsFindings confirmed the need for an online, couple-based intervention to support young couples experiencing RSH concerns after breast or gynecologic cancer. Qualitative themes suggested intervention preferences for: A highly flexible intervention that can be tailored to couples’ specific RSH concerns; Active steps to help members of a dyad “get on the same page” in their relationship and family building plans; A specific focus on raising partners’ awareness about how cancer can affect body image and physical intimacy; and Accessible, evidence-based information about RSH for both partners. These results, along with feedback from stakeholders, informed adaptation and finalization of the intervention content and format. The resulting virtual intervention, Opening the Conversation, includes five weekly sessions offering training to couples in communication and dyadic coping skills for addressing RSH concerns.ConclusionThe systematic adaptation process yielded a theory-informed intervention for young adult couples facing breast and gynecological cancers, which will be evaluated in a randomized controlled trial. The long-term goal is to implement and disseminate Opening the Conversation broadly to reach young adult couples with diverse backgrounds who are experiencing RSH concerns in cancer survivorship. Objective To determine whether presenting delivery room management options as defaults influences decisions to resuscitate extremely premature infants. Materials and methods Adult volunteers recruited from the world wide web were randomised to receive either resuscitation or comfort care as the delivery room management default option for a hypothetical delivery of a 23-week gestation infant. Participants were required to check a box to opt out of the default. The primary outcome measure was the proportion of respondents electing resuscitation. Data were analysed (...) using χ2 tests and multivariate logistic regression. Results Participants who were told the delivery room management default option was resuscitation were more likely to opt for resuscitation (OR 6.54 95% CI 3.85 to 11.11, p<0.001). This effect persisted on multivariate regression analysis (OR 7.00, 95% CI 3.97 to 12.36, p<0.001). Female gender, being married or in a committed relationship, being highly religious, experiences with prematurity, and favouring sanctity of life were significantly associated with decisions to resuscitate. Discussion Presenting delivery room options for extremely premature infants as defaults exert a significant effect on decision makers. The information structure of the choice task may act as a subtle form of manipulation. Further, this effect may operate in ways that a decision maker is not aware of and this raises questions of patient autonomy. The aim of this study was to investigate the impact of different coping styles on situational coping in everyday life situations and gender differences. An ecological momentary assessment study with the mobile health app TrackYourStress was conducted with 113 participants. The coping styles Positive Thinking, Active Stress Coping, Social Support, Support in Faith, and Alcohol and Cigarette Consumption of the Stress and Coping Inventory were measured at baseline. Situational coping was assessed by the question “How well can you cope with (...) your momentary stress level” over 4 weeks. Multilevel models were conducted to test the effects of the coping styles on situational coping. Additionally, gender differences were evaluated. Positive Thinking and Active Stress Coping had significant positive impacts on situational coping in the total sample. For women, Social Support had a significant positive effect on situational coping. For men, Active Stress Coping had a significant positive effect on situational coping. Women had higher scores on the SCI scale Social Support than men. These results suggest that different coping styles could be more effective in daily life for women than for men. Taking this into account, interventions tailored to users’ coping styles might lead to better coping outcomes than generalized interventions. (shrink)

I'm an experimental physicist. The basic physics research I do is funded primarily by the U. S. Government. As I write this, it is less than two weeks before the 1993 Presidential Inauguration. The new Clinton Administration is still of an unknown quantity. A new Presidential Science Advisor with excellent qualifications, Dr. John H. Gibbons, has just been appointed, but little is know about the science policies of the new administration.

Genetic testing has historically been performed in the context of chronic disease and cancer diagnostics. The timelines for these tests are typically measured in days or weeks, rather than in minutes. As such, the concept that genetic information might be generated and then used to alter management in the acute setting has, thus far, not been feasible. However, recent advances in genetic technologies have the potential to allow genetic information to be generated significantly quicker. The m.1555A>G genetic variant is (...) present in one in 500 individuals and predisposes to profound hearing loss following the administration of aminoglycoside antibiotics. These antibiotics are used frequently in cases of neonatal sepsis and it is estimated that approximately 180 neonates in the UK are at risk of antibiotic induced hearing loss each year because of this genetic change. Knowledge of this variant in the acute setting would allow clinicians to prescribe alternative antibiotics. The Pharmacogenetics to Avoid Loss of Hearing study will implement a genetic point of care test (POCT) for the m.1555A>G variant within two major UK based neonatal intensive care units. This represents the first trial of a genetic POCT aimed at altering management in the acute setting. This round table discussion outlines the novel ethical issues faced in the development of this trial and the legal barriers to implementation. We ask five stakeholders to provide their opinions on this trial and their perspectives on the concept of genetic testing in the acute setting.Trial registration numberISRCTN-13704894. (shrink)

Recent policy debates in the US over access to mental health care have raised several philosophically complex ethical and conceptual issues. The defeat of mental health parity legislation in the US Congress has brought new urgency and relevance to theoretical and empirical investigations into the nature of mental illness and its relation to other forms of sickness and disability. Manifold, nebulous, and often competing conceptions of mental illness make the creation of coherent public policy exceedingly difficult. Referencing a variety of (...) approaches to ethical reflection on health care, and drawing from the empirical literature on therapeutic efficacy and economic efficiency, we argue that differential rationing, ‘disparity,’ is unjustifiable. To better understand the available publications addressing ethical issues in rural health care we sought to identify the ethics literature that specifically focuses on rural America. We wanted to determine the extent to which the rural ethics literature was distributed between general commentaries, descriptive summaries of research, and original research publications. We identified 55 publications that specifically and substantively addressed rural health care ethics, published between 1966 and 2004. Only 7 (13%) of these publications were original research articles while (12) (...) 22% were descriptive summaries of research and 36 (65%) were general commentaries. The majority of publications examined (55%) were clinically focused while 27% addressed organizational ethics and 18% addressed ethical ramifications of rural health care policy at a national or community level. Our findings indicate that there are a limited number of publications focusing on rural health care ethics, suggesting a need for scholars and researchers to more rigorously address rural ethics issues. I was hit by a drunk driver in a pick–up truck while riding my motorcycle. Ironically, I was on my way to sell the motorcycle but never made it that far. I suffered a broken hand, femur, multiple lacerations and the most (...) severe injury of them all, the loss of my right leg just below the knee. I kept the fact that I owned a motorcycle from my parents by storing it at a friend’s house so I knew the news of my injury would be twice as surreal to them. I remember telling the ER nurse that I needed to be the one that called them to tell them what happened. She said she could not do that, but I told her that she did not know my mother and I was able to talk her into accommodating me.It was the summer between my junior and senior year of college and I had had just made Indiana University’s football team as a walk–on linebacker (without having played HS football) so life was good and there was pretty much nothing holding me back or anything I felt I could not do.In an instant the life I once knew would never be the same and I truly wanted to die but little did I know that life would get better, not worse. After keeping my lower leg attached with surgical tape for a week, the inevitable could not be avoided. It was devastating news. At first I refused to sign any papers authorizing the amputation surgery but I was told if I did not, I would surely die as gangrene had set in. I remember scribbling my signature and then throwing the pen—a doctor’s dream I’m sure but I have to believe they’ve seen it all before. I was in traction for the first six weeks and had lost 60 pounds. Two weeks after being removed from traction I was able to get upright but was extremely weak. The emaciated and ghostly white image of me was tough to look at in the mirror, especially with half my right leg missing. That really took some getting used to. At that point, it was like a bad dream and I wasn’t sure what to make of all of it but one thing for certain was that reality had sunk in. Surprisingly, it was at that point that I quit feeling sorry for myself and instantly knew what had to be done.I knew I needed help and I could not do it alone so with the help of my family and friends, I started to put goals in place for myself. First on the list was to gain some weight back so I resumed working out (mostly weightlifting) and turned to my mother’s [End Page 192] great cooking. There were some highs and lows during that process particularly with my first prosthetist (also an amputee) who told me my days of any type of sports activities “were over.” That was pretty devastating news to me but it didn’t take me long to realize he was only telling me things that he himself was never able to accomplish as an amputee and that just created more of an incentive for me because I knew deep down it was possible because I already had the drive and determination. Just under a year later I was back to a regular exercise program, my “pre–accident” weight and feeling healthy and in shape again.I have since become more appreciative of what I have. I used to box in college and in 1991, entered the Chicago Golden Gloves Super heavyweight (senior novice) division. Senior novice means you’ve had five or less fights at that time and that you were at least 21. I lost my first fight in 1991 on a controversial decision and came back in 1992 and had three fights. The fundamental insight of the Buddha, pratītyasamutpāda, is that, in the felicitous words of the Vietnamese Zen Master Thich Nat Hanh, things inter-are, in dynamic interdependence. It is because of the dynamic interbeing of things that genuine social and political freedoms are impossible without inner freedom. It was in his awakening to the truth of dynamic interbeing that, as Thurman describes it, "The Buddha found that inner freedom—freedom from our negative emotions and obsessive self-concern—is the essential precondition for goodness and social liberty" (p. 30).There is a tendency among Buddhist scholars to ignore or slight the social and political implications of wisdom and compassion. This is partly the result of reading modern Western individualism into Buddhist teaching and consequently focusing exclusively on how the individual person is transformed by wisdom and compassion, ignoring the social transformation inherent in the transformation of individuals. It is also partly the result of a failure to understand that compassionate action is as central to Buddhism as enlightened understanding. But Thurman clearly understands that the fundamental aim of Buddhist teaching and practice is to become a Buddha. And to become a Buddha, as Śāntideva says in the Bodhicaryāvatāra, a text that has inspired Tibetan Buddhists for over a thousand years, one takes the Bodhisattva vow to work tirelessly and unceasingly to bring happiness to all beings. In Śāntideva's own words: "As long as space abides and as long as the world abides, so long may I abide, destroying the sufferings of the world" (10.55). Compassion is inseparable from wisdom. John Hobson claims that throughout its history most international theory has been embedded within various forms of Eurocentrism. Rather than producing value-free and universalist theories of inter-state relations, international theory instead provides provincial analyses that celebrate and defend Western civilization as the subject of, and ideal normative referent in, world politics.
